Forms Needed for Adult Volunteers in Youth and Children Ministries

Below are the requested forms to be completed by adult volunteers working with Community Church of Waterford's Children, Junior High and Senior High Youth ministries. Forms are to be completed and returned to the ministry leader.

	Child/Youth Ministry Volunteer Form (Application and information form)
	Background Check Consent Form (Provides consent for the church to obtain a background check)
	Driver Certification Form (Needed for drivers who will be transporting youth or children from the church to planned off-campus activities or service projects).

Activities form for Junior High and Senior High Youth:

Information and Activities Consent Form (In order for youth to participate in youth ministry activities or trips, all pages of this form must be completed and signed by parent(s), legal guardian(s) and youth.)
